Partial List of Documents Candidates May Need To Produce for Background Investigation
- Birth Certificate (Must bring certified translation if not in English)
- Naturalization Certificate (if applicable)
- Social Security Card
- High School Diploma
- High School Disciplinary Records (Written Notarized statements explaining the disciplinary action)
- College Degree and transcript(s)
- College Disciplinary Records (Written Notarized statements explaining disciplinary action)
- Verification of Selective Service Registration
- Military Records (All DD-214s, Performance Evaluations, Disciplinary Records)
- Driver’s License with current address
- Certified Driver Abstract
- Registration for all vehicles presently owned, as well as household vehicles
- Insurance Card(s) for vehicle(s) (including ALL household vehicles)
- Letter(s) from Insurance Company regarding status of claims and standing
- ISO Insurance Report (C.L.U.E. report for Personal Property and Auto - 866-312-8076)
- Accident Reports WITH written statement(s) by you as to the circumstances of accident
- Parking Violations Bureau printout (Must bring statements regarding any summonses issued to you)
- Transit Adjudications Bureau (T.A.B.) printout - 347-643-5805
- Summons receipts (For ANY and Every Summons issued to you)
- Marriage Certificates (Must bring certified translation if not in English)
- Divorce Documents (Must bring certified translation if not in English)
- Children’s Birth Certificates (Must bring certified translation if not in English)
- Proof of Child Support Payments
- Documentation of ANY Past or Pending Criminal or Civil Litigation
- All Arrest(s) with documents and written statement by you as to the circumstances, age at time of arrest (ANY sealed records MUST be UNSEALED)
- Proof of Residence (Utility bill, Phone bill, etc.)
- W-2 Forms for the last five (5) years
- Any employer OUT OF BUSINESS MUST BE VERIFIED BY A W-2 OR SIMILAR PROOF
- If you were unemployed for ANY period of time, YOU MUST PROVIDE A STATEMENT AS TO HOW YOU SUPPORTED YOURSELF (parents, unemployment insurance, etc.)
- Social Security Abstract for the last seven (7) years (Work History)
- Written statement(s) by you for any DISCIPLINARY ACTION TAKEN BY AN EMPLOYER
- List of all debts (Provide statements for debt, both written statements and monthly statements)
- Credit Report from one of three reporting agencies (Equifax, Experian, of TransUnion) HARD COPY NOT FROM INTERNET (Not over 30 days old)
- All Licenses (Pistol, Rifle, Hack, Hunting, etc.)
- Proof of Weapons (YOU MUST PROVIDE MAKE, MODEL, SERIAL#, AND TYPE)
- Civil Service test results indicating date, exam# and list# (Must provide written statements regarding the outcome of all exams)
- Five (5) references (No immediate family)
- Spouse/Partners occupation, employer, and salary
- Work Record Release Information (Contact person’s name, telephone number, and location of records)
- Candidate Financial Data Sheet
- Candidate Chronological Data Sheet(s)
